Please fill in this form with your preferences for your hook order. Once I receive it I will contact you on Instagram (if provided) or email to finalize the design.

You must fill out this form for each different hook you would like to order. I will group the order together into one package and ship when all your hooks are completed unless otherwise specified. You will pay shipping only once per shipment.

Allow up to 6 weeks for production (Most should be completed much sooner, but I have limited time to work on these so it can get delayed). If you need a rushed order, please note that in the “Describe Special Pattern or Topper” section.

Once your hook is complete, I will send you an invoice via Square to the email provided to request billing and shipping information.

Shipping will be $5 to U.S. destinations and starts at $15 to international destinations. International customers mat be responsible for VAT upon receipt of their items.
